Mater Dei School

As in past years, students in Grades 1-8 for the 2019-2020 school year need to have a Medical Evaluation Form and current Immunization Form completed and returned to Mater Dei by the first day of school (i.e., Monday, August 26, 2019).  

Students requiring Epi-pens and/or inhalers also need to have those respective forms completed.
